Description: Aims:
To increase knowledge of the NHS commissioning landscape and, by understanding the local health needs, be able to apply effective consultation and clinical skills combined with therapeutic knowledge and laboratory test interpretation to inform shared decision-making and improve patient outcomes. Through audit of practice, medicines review, medicines reconciliation and effective multidisciplinary working students will be able to optimise patient care and improve efficiencies within their primary care setting.

Learning Outcomes:
1. Discuss and evaluate the NHS structure and commissioning landscape within primary care, the roles of the practice team within the wider primary care team and reflect on your own development needs; define the local practice population. (portfolio tasks)
2. Develop professional relationships within the multidisciplinary team, across sectors and professions, to improve communication and patient outcomes. (Case Based Discussion[CBD], Clinical Activity Reflective Exercise [CARE])
3. Use clinical IT systems to effectively document actions, audit prescribing practice and communicate with the primary care team to influence and support prescribing practice and improve patient outcomes and NHS efficiencies. (Medicines Optimisation Tool [MOT])
4. Apply leadership appropriately within a healthcare setting and identify ways to strengthen leadership skills. (MOT)
5. Demonstrate effective patient centred consultation skills (which meet relevant practice standards) and apply motivational interviewing to enhance the patient experience. (Observed Structured Clinical Examination [OSCE])
6. Take a comprehensive medication history and appropriately reconcile medication. (OSCE)
7. Demonstrate good medication review and promote medicines optimisation to improve patient outcomes and reduce waste. (CBD)
8. Identify and interpret key laboratory tests and apply this to monitor therapy and inform prescribing decisions. (MOT)
9. Demonstrate sound knowledge and application of commonly used clinical skills i.e. chest examination, BP measurement, urine analysis and blood glucose testing. (OSCE)
